For your new patient appointment there will be a few forms that we will need you to fill out. One of these forms can be found on our website under New Patients > Online Forms. You have the option of either printing it out and bringing it to your first appointment with you, or you can fill it out and submit it online. We also have some in the office for you to fill out if that is easier for you, and we can assist you if needed.
Next, Dr. Dale will take you into the treatment to discuss your health, your concerns, and your treatment options. Dr. Dale will take time to learn more about you, your condition, and your expectations in order to determine how he can help you meet your goals with chiropractic care.
After getting to know you Dr. Dale will perform a complete chiropractic examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. However, nothing will be done in our office without your consent.
Depending on your condition, we may need to have x-rays done as well. Dr. Dale has a relationship with the Mayo Clinic, and can send you there to get images taken if needed. X-Rays help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Most people are amazed once they see their x-rays up close. In some cases, they can even identify their misalignments and degeneration themselves.
Report of Findings
Once all of the information is collected and examinations are performed, Dr. Dale will give you a detailed report of all the findings and answer any questions including:
- How can you help me?
- How often do I need to come in?
- What will my treatment cost?
Dr. Dale will discuss his recommendations with you and notify you if your condition requires care with other providers. Dr. Dale will provide the best treatment and wellness program for your needs.
At the conclusion of your first appointment you, can chose to experience your first chiropractic treatment. This may include spinal adjustments, physical therapy, and/or soft tissue massage. Treatment is interactive, so you can express concerns about the different styles of treatment at any time.
Prior to leaving, the doctor will suggest a wellness program to incorporate outside of treatment here at the office. If you are in pain when you first come into our office this may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches. If you desire he will also work with you to create health habits and routines for your lifestyle. Every person is unique, therefore everyone requires a customized wellness plan. The purpose of our wellness program is for you to achieve good spinal alignment, have a healthy diet, exercise, and maintain a positive mental state.
